Message (optional)	E.g. "My dog was doing flips today. He is the greatest dog in the world."
File (Photo or Video)	E.g. "dog.gif". Image formats supported: GIF, JPG, PNG, TIFF. Video formats supported: AVI, MOV, WMV, MPG, MP4, ASF, 3GP, 3G2.
*Your Name*	Your name will not be published and is kept for record keeping only in the event that your submission is chosen as the Dog of the Month.
*Your Email*	Your email address will not be published and is used for record keeping only. You will be notified by email if your dog is chosen as the Dog of the Month.
